Anirtion Road, Idaho Falls, Idaho, and Rehab Serlik,':es of Idaho, hereby known as Contractor, is 'for tile school year 2014-2015. The following service(s) requested: Speech and Language Date of set -vice: beginiling october:M, 2014through Ame 30, 2015 Hours of service: at the maximmia of 30 hours per week at the rate of $50 per licau, Contract fee not to exceed $42,0M Contractor requirements attached hereto are made a part of this contact. The parties agree that the Contactor is solely responsible for all costs and expenses incurred in connection with tire performance of those services described in this agreement unless noted above. 1, The Contractor agrees to provide speech/language pathology and related services in accordance with the rules and regulations established by the Idaho State Board of Education as provided in Section 33-3003, Idaho Code, as amended for exceptional Students living in this district. 2, The Contractor agrees to provide education and/or related services for identified students(s) including: a. Tile appropriate staff and oversight to implement IEPs for each student as determined by the school IEP tearn; 1). Services to the Students according to the school districts 2014-2001.5 school calendar (attached), excluding emergency school closures or when student is absent, 3. The Contractor further, agrees to provide the District tile following: a. Services as authorized in the students IEP/504; b, Other services such as consultations and meetings; c. Assurance that all work will be performed in accordance with the highest professional standards; d. A copy of the set -vice provider's Staff Qualifications showing professional credentials for the districts files; e. Verification all eniployees who come into contact with tile StUdent have been subject to a criminal background check as required by the Idaho Code 33-1,30 and have been determined to not have a criminal history inconsistent with working with children. f. The Contractor and DimLhci agrees to comply with all pertinent statues mfthe State of Idaho and such rules and regulations asthe State Board of Education may legally prescribe, which are by reference incorporated in and made a part of this Contract esthough set forth herein. 6, The District assures that health-related services or program placement will begin after having conducted an IEP meeting todevelop mn IEP. The Contractor, atthe District's discretion, may request or attend subseClUent IEP team meetings to revise t1rie student's IEP, the a District representative Must participate inall such meetings. * The District will pay the Contractor based on submission of an invoice with 600um^ntahnn as described in3.h. 8. The district will provide documentation necessary for the Contractor iocarry nutthe portion of the IEP that falls under the Contractor responsibility. 9. The District may terminate the Agreement immediately if the District determines that Contractor has Willfully violated any statUtory requirement or government regulation or the services related to this Agreement are modified or terminated for a Student. Either party may terminate this AgreementWithout Cause upon providing 30days written notice tothe other party. 10. Contractor shall hesolely liable for any losses ordamagesresulting from performance ofany ofthe services covered bythis Agreement. Contractor shall inJemnifyand hold harmless the District from any liability, including, but not limited to, cost, expenses, and attorney fees, resulting from Conti -actor's performance ofthe services provided user this Agreement. Rehab Services of Idaho also agrees iomaintainliability insurance inthe minimumam*untw| $1,OOO,OOO.0Oand worker's compensation coverage for its employees. |fRehab Services mfIdaho does not have worker's cornpensation, Rehab Services of Jdaho will pay $ 55 for every $100 billed. This will be deducted in the invoice.
