private line connecting 2125 Monticello Drive Idaho Falls Idaho
83404 to the existing port at the Ammon Central Office.
Monthly discount for having multiple services with Silver Star
Communications
SubTotal with 5 year term agreement
One -time Construction fee of $12,500.00 will appear on first
billing statement. This payment will be applied as a credit
towards the monthly service. There will be no monthly bill for
months I through month 31 until that credit is used up.
Starting on month 32 the normal monthly rate of $399:00 plus
fax will resume.
This agreement is conditional on receiving final approval from a
Silver Star Communications Engineer that the construction
costs are reasonably in line with the services selected.
Early termination: Customer does have the option to move or
transfer the services to a new address without being in violation
of the term agreement. At the end of the 5 year term
agreement services will continue at the speed and the same
price on a month to month basis. Should services be canceled
prior to the completion of the agreed term listed in quote, early
termination charge will apply in the amount of the remaining
monthly charges, due 30 days after services are terminated.
