The Distnct hereby engages the Provrder to provide servrces descflbed herein under "Scope and Manner
of Services" forthe2015-2016 E-Rate fundrng cycle The Provider hereby agrees to provrde lhe Dislnct with such

Scope ol Available Servicsa
Comprehensrve e-rate servrces, rncluding
Completing all application forms fiIngs and requlred correspondence
Maoaging, filing and storing all relevant paperwork associated wrlh your application
Being the point of contact for all tevieyvs. audits and appeals
Answering all Program lotegflty Assurance (PlA) questions
Revrewing Free & Reduced lunch ligures and provrding gurdance to maximrze drscounts
Answering complrance queshons and ellgibllrly rssues
Preparing and lollowing-up with servrcs substrtut,ons and SPIN number changes
Following-up with rnvorclng payments and dlscounls
Answenng questrons concernrng polrcres
e-rate appeal process, rncluding:
Prepaflng and respondrng lo Selectrve Review Audits
Preparing and Filing SLD and FCC Appeals
Technology plannrng services. includrng
Assisting with developrng technology plan consistenl with TEA requrrements
Payment for SerYice8 Rcndered
Flat rate of S5000 annually for comprehensrve erate services, with e-rate appeal and/or technology pianning billed
at the additronal rate of 9100 per hour iAny work necessary to complete lhe 2014-2015 cycle will also be brlled
at lhe rate of 5100 per hour )
Should the Orstrict lail to pay the Provrder lhe full amcunl specrfied in any invoice within 30 calendar days
of the rnvoice's date, a lale fee equal io $50 00 shall be added to the amounl due
Thrs contract shall be governed
