Proposal for Nutrition Consulting & Meal
Planning
Scope of Work
This agreement is entered into by the Bonneville Joint School District #93 -Child Nutrition
Department 2461East24lh North Idaho Falls. ID 83401. herein referred to as the Contractor and
Kristopher Crane RD. LP -156 Mayrice Street Noah Apartment 214 Twin Falls ID. 83301. herein
referred to as the Registered Dietitian. The purpose of this agreement is for consultative
services of the Registered Dietitian for the Bonneville Child Nutrition, and is subject to the
terms and conditions as follows:
Need for Consulting
1. The contract will begin September 1"2017 and end on June 8th 2011!.
2. The Registered Dietitian will review the current school menu and make
modifications as needed for specialized allergies/intolerances such as, but not
limited to: Gluten Free, Lactose Free, Ketogenic Diet, Peanut & Nut Free, and others
as deemed necessary by the Director of Child Nutrition to meet the needs of the
students and parents of District 93. The Registered Dietitian will use the current
Bonneville Child Nutrition Inventory to make these modifications, and can make
product recommendations, if needed. The specialized menus drafted will be for five
days a week (Monday-Friday) cycle menu, and will meet the standards of the USDA
School Nutrition Requirements outlined under the current National School Lunch
Program Regulations (http://www.fns.usda.gov/nslp/regulations).
3. The Registered Dietitian will make recommendations on new products, equipment,
and tools that he has found useful and helpful in meeting the needs of the
specialized menus and submit them to the Bonneville Child Nutrition Director,
Heather Plain SNS.
4. The Registered Dietitian will be available to travel to the various school locations to
educate and train service staff in the area of dietary services, sanitation, as well as
be available to the site managers for consultation on meeting the needs of the
specialized menus and nutrition guidance .
.5. The Registered Dietitian will work with the Director of Bonneville Joint District 93
Child Nutrition as the primary contact for nutrition education, counseling, training,
and specialized menu development as required by the Director of Bonneville Child
Nutrition.

Qualifications
1. The Registered Dietitian will have received the Registered Dietitian (RD) or Registered
Dietitian Nutritionist (RON) credential from the Commission on Dietetic Registration
(CDR) and will maintain that credential as outlined by the CDR.
2. The Registered Dietitian will meet and keep the standards of the Academy of Nutrition
and Dietetics by using the most current evidenced-based research, and nutrition
counseling practices as outlined by the Academy's Standards of Practice (SOP) and
Standards of' Professional Performance (SOPP). The Registered Dietiti<>n will also meet
and keep the standards held by the Idaho State Board of Medicine for State Licensure as
outlined by the Idaho State Board of Medicine Regulations.
3. The Registered Dietitian will provide their own liability Insurance Coverage with proof
of Insurance to be kept with the Registered Dietitian and a copy to be kept with the
District Manager of Bonneville Joint School District 93 Child Nutrition Department as
requested.
4. The Registered Dietitian will also have sufficient experience (2+ years) working with
specialized menus, including those found in Hospital Patient Nutrition and School
Nutrition Services, to meet the expectations of Bonneville Joint School District 93 Child
Nutrition Department.

agreement.
Price
1. Compensation for consulting services will be at the rate offortv dollars !$40.001 per
consultjng hour and wm not exceed 20 hours per month without prior approval of the
Director of Bonneville Child Nutrition Department. Pursuant to this agreement, the
Registered Dietitian will be responsible for all tax liabilities, insurances, and cost of
travel. The Registered Dietitian recognizes that he is an independent contractor and wil I
keep all required licenses and insurances. The Registered Dietitian agrees that
termination of any required licenses/certifications/insurances as recognized by the State
of Idaho for whatever reason, will automatically be justifiable cause for termination of
this agreement on the same date of loss of said assurances without written notice being
tendered by the Bonneville Joint School District Child Nutrition Department.
Confidentiality
1. All records and other information required by the Registered Dietitian regarding the
individuals/agencies receiving services under this contract are confidential and will
be maintained and protected by the Registered Dietitian to ensure confidentiality.
The Registered Dietitian will not disclose such information unless authorized by the
Director of Bonneville Joint School District 93 Child Nutrition Department.

Other Terms/Conditions
1. Amendments to this agreement will be recognized when documented in writing, signed
as approved by the Director of Bonneville Joint School District 93 Child Nutrition
Department and the Registered Dietitian; and attached to this Proposal for Nutrition
Consulting and Meal Planning.
Acceptance of Agreement
Therefore, by signatures set forth below, the parties do hereto mutually agree to perform the services
described in this agreement pursuant to the terms set forth herein.
