FOOD SERVICE CONTRACT
Bonneville Joint School District No.93 and Monticello Montessori School
School Year 2015-2016
This agreement is made and entered into between Bonneville Joint School District f93 (District 93) and
Monticello Montessori School (MMS). WHEREAS the District #93 agrees to supply meals for the rates
herein listed:

It ls further agreed that District 93 will follow the regulations and coordinate with MMS the
implementation of the provisions of the NSLP Food Service Program.
FREE AND REDUCED APPLICATIONS: MMS will receive the Free and Reduced Applications, verify that the
applications are filled out correctly and pre-approve each application according to the USDA guidelines.
District 93 will then receive the applications and finalize the approval process for the Free and Reduce
Applications and mail letters to the households. District 93 will be entitled to utilize this information to
request federal funding and reimbursement.
STUDENT DATA: MMS will send a student data file in .csv format weekly to District 93 via portable USB
drive. This information will be used for state reporting purposes as well as keeping student information
current and accurate. MMS will notify District 93 immediotely of ony students tronsferring from District
93 to MMS to prevent duplicote students in the POS dotobose.
LUNCH MONEY: MMS will collect the lunch money received from students and keep a record of the
student accounts. Upon receipt of invoice from District 93, MMS will pay the invoice within 15 days.
MEAL COUNT: MMS will call in lunch counts daily to Woodland Hills Elementary School (WHES) by 8:45
a.m. (Lynette Johnson, Manager 227-9826). WHESstaffwill prepa re the food a nd be ready to serve the
MMS students at approx. 12:10 each day. MMS will provide the followinB staff: 1 person to wipe and
put away tables and monitor the cafeterla and 1 additional person will be required to help monitor
students in the halls and cafeteria.
All MMS students will eat on site at Woodland Hills Elementary. No food will be transported to MMS for
any reason. This agreement is contingent on the enrollment at Woodland Hills Elementary. lf for any
reason the principal at Woodland Hills can no longer accommodate the additional students during the
lunch period, this agreement will become invalid and will need to be renegotiated.

SCHOOL CALENDAR: District 93 agrees to provide lunches on the days that are in session and serving
lunches to students within the school district. Days that District 93 is not in session or not providing
lunches, MMS will be responsible for providing its own lunch program. MMS is not required to purchase
lunches on days it is not in session. MMS will provide District 93 with a school calendar for fiscal year
2075-2016.
ADMINISTRATIVE COSTS:
At the end of the school year MMS will be billed 51,50o for Administrative Costs and 52,500 for a
cafeteria employee hired by District 93 to assist with the extra work required for these additional meals.
It is understood that District 93 cannot operate this contract at a financial loss. For this reason the
ongoing revenues and expenditures will be examined to determine the financial feasibillty of continuing
to provide food senvice to MMs.
Should there be financial losses incurred by Distrlct 93; MMS will be glven an opportunity to supplement
the current revenues to the extent that the costs of the program are paid. Should MMS decide not to
subsidize current revenue; District 93 will cancel the lunch program immediately upon written notice.
This agreement shall be effective 8/3L/15- 612/76 and may be terminated with 30 days' notice by elther
pa rty in writing.
