HomeMy WebLinkAbout2380 Homebound, Hospital, and Home InstructionSTUDENTS 2380 Page 1 of 4 Bonneville Joint School District No. 93 2380 Homebound, Hospital, and Home Instruction HOMEBOUND, HOSPITAL, AND HOME INSTRUCTION Bonneville Joint School District No. 93 supports the provision of homebound, hospital, and home instruction for eligible students who are unable to attend school due to illness, accidents, or unusual disabling conditions relating to health, subject to the conditions of this policy. 1. Eligibility a. Students must be unable to attend school due to illness, accidents, or an unusual disabling condition relating to health. b. The student must have a medical condition which will cause an absence from school of ten (10) or more consecutive days. c. The student must reside within District boundaries. d. The student must be registered in a District school. 2. Request and Documentation a. A parental request for Homebound Services must be submitted using form 2380F1 Parents Request for Homebound Services. b. A recommendation for Homebound Services by a licensed medical Physician must be provided on form 2380F2 Medical Requests for Homebound Services. c. A diagnosis by a Physician indicating whether there is any danger to, or caution for, the Homebound Supervisor to exercise must be submitted. d. The attending Physician's estimate of the length of time the student may be absent from school must be provided. e. The student's history of previous absences and grades maintained must be reviewed. 3. Approval a. In all cases, Homebound services will be provided based upon individual needs and must be approved by the Principal, Director of Special Services, and his/her designee (the Homebound Supervisor). Appropriate educational services may begin as soon as eligibility has been established through the Director of Special Education/designee and coordinated with the building administrator of the student. 4. Service Provision a. The Homebound Program does not take the place of the classroom or the school's responsibility; rather it serves as a bridge to aid the student's return to the school setting. b. A teacher or an aide will be provided in the home or local hospital within the District's boundary. c. The Homebound Supervisor waits until teachers have issued homework assignments before scheduling an appointment, which may cause a delay between the time the homebound paperwork is submitted and the actual time the Homebound Supervisor contacts the student. d. Homebound Services for Lincoln Alternative High School will be provided through correspondence courses or the internet. 5. Restrictions a. While on Homebound status, the student may NOT attend, or participate in, any extracurricular school functions (dances, sports, parties, productions, etc.) except when special permission is given from both the School Principal and the Homebound Supervisor. 6. Students with IEP and 504 Plans a. For students with Individualized Education Programs (IEPs) or 504 plans who require homebound, hospital, or home instruction, all services and accommodations outlined in their existing plans will be maintained to the greatest extent possible during the period of homebound instruction. b. The student's IEP or 504 team will convene to review and potentially revise the student's plan to ensure appropriate accommodations and services are provided during homebound instruction. c. The Homebound Supervisor will collaborate with the student's special education case manager or 504 coordinator to ensure continuity of services and implementation of all necessary accommodations. Upon the student's return to the regular school setting, the IEP or 504 team will reconvene to assess the student's progress and make any necessary adjustments to their plan. e. If the period of homebound instruction is expected to be extensive, the IEP or 504 team may consider revising goals or services to best meet the student's needs in the homebound setting while maintaining compliance with IDEA or Section 504 requirements. 7. Special Cases a. In cases where it is determined that it would be detrimental for a student to attend school with his/her peers, the Homebound Program may be instituted at the discretion of the Superintendent or the Board of Trustees. 8. Attendance Calculation a. Students receiving homebound services shall be included in calculating the average daily attendance. 9. Exclusions a. Homebound services will not be provided under the following conditions: i. As a means for completing graduation requirements for students not desiring to attend school; or ii. If it is determined that the student is engaged in employment or is in other ways exhibiting capabilities of attending school; or iii. If the estimated time frame for Homebound Service expires, and a "Time Extension Form" has not been resubmitted by the Medical Physician, and returned to the Homebound Supervisor. DEFINITIONS Homebound Student: A student who would normally and regularly attend school, but is confined to home or hospital because of an illness or accident for a period of ten (10) or more consecutive days.
