IMMUNIZATION REQUIREMENTS
Immunization records or a valid exemption are required for student admission to the
Bonneville Joint School District No. 93 schools.
Guidelines
1. At the time of first admission to any public school within Bonneville Joint School
District No. 93, and before attendance, all students must present:
a. An immunization certificate statement signed by a physician or a physician’s
representative stating the type, number, and dates of immunizations received; or
b. A valid medical, religious or personal exemption reco rded on the Idaho School
Immunization Requirement Exemption form #3004F1.
2. An exemption form is not valid if the parent/guardian has forgotten or lost their
child’s immunization record.
3. District principals/designees may access the voluntary statewide registry of
immunization status maintained by the Idaho Department of Health and Welfare to
verify whether students have received the required immunizations.
Required Immunizations
All Students attending school in the District are required to have the immunizations
listed in exhibit #3004E2 Summary of Immunization Requirements.
Conditional Admission: Schedule of Intended Immunizations
1. A statement upon the Idaho Conditional Admission to School: Schedule of Intended
Immunizations form #3004F2 must be received from a parent/guardian of any
student who is not immunized, does not qualify for an exemption, and who is in the
process of receiving, or has been scheduled to receive the required immunizations.
2. The statement must be provided to the school at the time of first admission and
before attendance providing the following information:
a. Name and birth date of the student;
b. School attending and grade in which the student is enrolled;
c. Type, number, and dates of immunizations to be ad ministered;
d. Signature of the legal parent/guardian providing the information; and

e. Signature of a physician or physician’s representative.
3. If a student is admitted to school and fails to continue the schedule of intended
immunizations, that student will be excluded from school until documentation is
presented setting forth the administration of the required immunizations or an
exemption.
Exceptions from Immunizations
A student who supplies documentation to the school of one (1) or more of the following
conditions is not required to undergo the required immunizations:
1. Laboratory proof of immunity to any of the childhood diseases set forth on the
Idaho School Immunization Requirement Exemption form #3004F1; or
2. Disease Diagnosis. A student who has a statement signed by a licensed health
care professional stating that the student has had varicella (chickenpox) disease
diagnosed by a licensed health care professional upon personal examination, will
not be required to receive the immunization for the diagnosed disease. or
3. Suspension of Requirement. The Regulatory Authority may temporarily suspend
one (1) or more of the immunization requirements listed in Section 100 of IDAPA
rule 16.02.15, if the Regulatory Authority determines that suspension of the
requirement is necessary to address a vaccine shortage or other emergency
situation in the state. The Regulatory Authority will suspend a requirement for the
length of time needed to remedy the vaccine shortage or emergency situation.
Exemption from Immunizations
A student who supplies documentation to the school of one (1) or more of the following
conditions is not required to undergo the required immunizations:
1. Life or Health Endangering Circumstances. A signed statement of a licensed
physician that the student’s life or health would be endangered if any or all of the
required immunizations are administered.
2. Religious or Other Objections. A signed statement of the parent, custodian, or
legal guardian that must be either:
a. On a standard Department form or similar form provided by the school (Idaho
School Immunization Requirement Exemption form #3004F1); or
b. A signed statement that must include:
1) The name of student, and the student’s date of birth; and

2) A statement indicating that the student is exempt from immunization as
provided in Section 110 of this rule for religious or other objections; and
3) The signature of the parent, custodian, or legal guardian.
Exclusion from School
1. The Bonneville Joint School District No. 93 will deny attendance to any student in
preschool (pre-K), or kindergarten through grade twelve (K-12) not in compliance
with this policy.
2. Any student denied attendance will not be allowed to attend any schools w ithin the
District until he/she is in compliance with this policy.
3. Students who are diagnosed or suspected of having a contagious or infectious
disease or students who have been exposed to a contagious or infectious disease
for which they have not been immunized will be excluded from school.
4. The district will also close school on order of the state board of health or the local
health authorities.
Reporting Requirements
A copy of a report of the immunization status in required grades will be submitted to the
Department of Health and Welfare on or before November 1 of each year.
DEFINITIONS:
Conditional Admission to be eligible for conditional attendance, a child must have
received at least one dose of each required vaccine and currently be on schedule for
subsequent immunizations.
Exemption is the process of freeing oneself from a rule or an obligation imposed on
others.
Medical Exemption is when a child has a medical condition that prevents them from
receiving a vaccine.
Nonmedical Exemptions is for religious or philosophical reasons.
