STUDENTS <br />BIRTH CERTIFICATE <br />3002 <br />Page 1 of 2 <br />The Bonneville Joint School District No. 93 Board of Trustees, in compliance with Idaho <br />Code section 18-4511, requires that children meet identification requirements upon <br />enrollment for the first time in elementary or secondary school in the District. <br />This requirement is subject to provisions of the McKinney Homeless Assistance Act. <br />(Policy #3060 Education of Homeless Children). <br />Guidelines <br />Certified Birth Certificate Requirement <br />A certified birth certificate issued from the Bureau of Vital Statistics, or other legal proof <br />of a child's age, shall be required of students enrolling in the Bonneville Joint School <br />District for the first time. <br />1. The certificate must be visually reviewed by school personnel. <br />2. A copy will be made and placed in the student's official file. <br />3. A student transferring schools within the District need not provide proof of identity <br />and birth date if the student's record already contains such verified information. <br />Other Reliable Proof <br />1. Other reliable proof of the student's identity and birth date may include: <br />a. a passport, <br />b. visa, or <br />c. other governmental documentation of the child's identity. <br />2. Other reliable proof does not include hospitals' records, bank records, baptism <br />certificates, etc. <br />3. Other reliable proof must be accompanied by an affidavit explaining the inability to <br />produce a certified birth certificate. <br />Notification <br />1. If a birth certificate or other reliable proof of the student's identity and birth date is <br />not provided, the building principal/designee shall notify the person enrolling the <br />Bonneville Joint School District No. 93 <br />
