3006 <br />Page 1 of 2 <br />STUDENTS <br /> <br />PROOF OF RESIDENCY <br /> <br />Unless open enrollment status is granted by the Bonneville Joint School District No. <br />93, students who enroll in District schools must reside full-time in the School District <br />boundaries with their natural parent or legal guardian. Students and their <br />parent(s)/guardian(s), not granted open enrollment status, must remain full-time <br />residents of the District for the entire period of enrollment in District schools. <br /> <br /> <br />Guidelines <br /> <br />Proof of Residence <br /> <br />1. Proof of residence may be required when a student initially enrolls in a school and <br />whenever a change of residence occurs. <br /> <br />a. Proof of residency records must include the name and street address of the <br />parent/guardian. <br /> <br />b. The property address given must be the actual location where the student and <br />parent/guardian live full-time. <br /> <br />2. The principal/designee shall accept any of the following records as proof of <br />residency: <br /> <br />a. A current residential property tax statement or deed, a lease or rental <br />agreement consisting of written evidence that the agreement is valid and <br />current, or a current utility bill (gas, electric, water, home telephone, or cable), <br />or vehicle registration; OR <br /> <br />b. A third-person affidavit of residency completed with the school district <br /> <br />(1) The notarized affidavit shall be completed and signed by the parent/guardian, <br />as well as the legal owner or lesser of the property where the student and <br />parent/guardian reside. <br /> <br />(2) The affidavit will be in effect until the parent/guardian provides the required <br />proof of residence, but no longer than the end of the current school year. <br /> <br />Verification of Residency <br /> <br />A District representative may visit the address given by any parent/guardian to verify <br />residency. <br /> <br /> <br /> <br /> <br /> <br /> <br />Consequences for Violating the Policy <br />
