IF YOU ARE INJURED ON THE JOB:
Administer Frist Aid as necessary
Notify your Supervisor immediately
Complete all necessary forms
Contact District Human Resource Benefits Specialist

The Emergency Room will only be utilized for emergency conditions or after hours.
Any Emergency Room visit must be authorized by your Supervisor/Safety Manager first.
COMPLETE ALL NECESSARY FORMS
Forms must be completed during the shift of the accident
Return all necessary forms to Benefits Specialist within forty-eight (48) hours
If you seek treatment outside the authorized providers, you could be held responsible
for the associated medical bills.
